Acc 612 was given to the Contemporary Medical Archives Centre by Dr C I Pelton, of the Health Centre, Victoria Avenue, Wellington, via Dr Michael Bevan of the Wellcome Unit for the History of Medicine, Oxford, in January 1996 and Acc 1235 directly by Dr Pelton to Archives and Manuscripts in April 2004.

Patient record envelopes (male and female), 1920s-1980s, with brief notes of consultations, from practice in Wellington, Somerset. Prior to 1948 they include details of insurance society of patient.

There was no obvious arrangement to this material. It has been grouped chronologically according to when the folders were opened.
